• Adults with treatment-resistant depression (TRD)
• Depressive symptoms in adults with major depressive disorder (MDD) with suicidal thoughts or actions

If you have questions about completing the form or cannot access it, please contact our Care Team at 877-232-2857.
Your physician can fax the referral form to our office at 269.250.8020.

During our call, we’ll ensure your patient information is complete and up-to-date, discuss the treatment, schedule your appointment, and answer any questions.
